PHOTO RELEASE

I hereby grant and authorize Family Christian Center/Refuge Productions the right to take, edit, alter, copy, exhibit, publish, distribute and make use of any and all pictures or video taken of me by Family Christian Center/Refuge Productions to be used in and/or for legally promotional materials including, but not limited to, newsletters, flyers, posters, brochures, advertisements, fundraising letters, annual reports, press kits and submissions to journalists, websites, social networking sites and other print and digital communications, without payment or any other consideration. This authorization extends to all languages, media, formats and markets now known or hereafter devised. This authorization shall continue indefinitely, unless I otherwise revoke said authorization in writing. 

I understand and agree that these materials shall become the property of Family Christian Center/Refuge Productions and will not be returned. 

I hereby hold harmless, and release Family Christian Center/Refuge Productions from all liability, petitions, and causes of action which I, my heirs, representative, executors, administrators, or any other persons may make while acting on my behalf or on behalf of my estate. 

If the person signing is under the age of consent, then this release must be signed by a parent or guardian, as follows: 

I hereby certify that I am the parent or guardian of named above, and do hereby give my consent without reservation to the foregoing on behalf of this individual.

COVID-19 Statement

PLEASE READ CAREFULLY BEFORE CHECKING

I UNDERSTAND I acknowledge the contagious nature of the Coronavirus/COVID-19 and that the CDC and many other public health authorities still recommend practicing social distancing. 

I further acknowledge that Family Christian Center/Refuge Productions, Inc. have put in place preventative measures to reduce the spread of the Coronavirus/COVID-19.

I further acknowledge that Family Christian Center/Refuge Productions, Inc. can not guarantee that I will not become infected with the Coronavirus/Covid-19. I understand that the risk of becoming exposed to and/or infected by the Coronavirus/COVID-19 may result from the actions, omissions, or negligence of myself and others, including, but not limited to, staff, members, volunteers and their families.

I voluntarily serve Family Christian Center/Refuge Productions, Inc. and acknowledge that I am increasing my risk to exposure to the Coronavirus/COVID-19. I acknowledge that I must comply with all set procedures to reduce the spread while attending service. I will wear my mask on stage and backstage.

I attest that:

* I am not experiencing any symptom of illness such as cough, shortness of breath or difficulty breathing, fever, chills, repeated shaking with chills, muscle pain, headache, sore throat, or new loss of taste or smell.

* I have not traveled internationally within the last 14 days.

* I have not traveled to a highly impacted area within the United States of America in the last 14 days.

* I do not believe I have been exposed to someone with a suspected and/or confirmed case of the Coronavirus/COVID-19. 

* I have not been diagnosed with Coronavirus/Covid-19 and not yet cleared as non contagious by state or local public health authorities.

* I am following all CDC recommended guidelines as much as possible and limiting my exposure to the Coronavirus/COVID-19.

I hereby release and agree to hold Family Christian Center/Refuge Productions, Inc. from, and waive on behalf of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to myself and/or property that may be caused by any act, or failure to act of the church, or that may otherwise arise in any way in connection with any services received from Family Christian Center/Refuge Productions, Inc. I understand that this release discharges Family Christian Center/Refuge Productions, Inc. from any liability or claim that I, my heirs, or any personal representatives may have against the salon with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to Family Christian Center/Refuge Productions, Inc. This liability waiver and release extends to the church and production company with all owners, volunteers and employees. 

Social Media Statement

Family Christian Center and Refuge Productions understands that social media is a part of our culture, communication and means to distribute information. As a staff member, team member, volunteer or someone who serves on a department I agree to the following:

I agree that I will not post inappropriate photos or videos of myself or others that are sexually provocative and inappropriate in nature. 

I am aware that Family Christian Center and Refuge Productions are faith-based organizations that do not condone drug and alcohol use. I agree that I will not post photos or videos of illegal or legal substances that do not uplift the values of these organizations. 

I agree that I will not slander; the action or crime of making a false spoken statement damaging to a person's reputation or libel; a published false statement that is damaging to a person's reputation, a written defamation

I agree that I will not post statements or videos that encourage hate speech or actions towards another person based on race, age, gender, or religious beliefs 

I agree to represented myself and Family Christian Center and Refuge Production with Christian morals, ethics and values.

WAIVER, RELEASE OF ALL CLAIMS AND HOLD HARMLESS AGREEMENT

As a participant in these activities, I recognize and acknowledge that there are certain risks of physical injury, and I agree to assume the full risk of any injury (ies), damage(s) or loss (es) which I may sustain as a result of participating, in any manner, in any and all activities connected with or associated with such activities or programs.  I further recognize and acknowledge that all unruly behavior with other participants is potentially hazardous and involves substantial risk, and may result in injury.

I agree to waive and relinquish any and all claims I may have against FAMILY CHRISTIAN CENTER and/or REFUGE PRODUCTIONS as a result of participating in these activities, as well as any and all other persons and entities, of whatever nature, that might be directly or indirectly liable for any injuries that I might sustain while participating in these activities or programs.  (The parties described in the preceding sentence are referred to as “released parties” in the remainder of this agreement.)

I do hereby fully release and discharge FAMILY CHRISTIAN CENTER and/or REFUGE PRODUCTIONS of any and all claims for injury(ies), damage(s) or loss(es) which I may have or which may occur to me on account of my participation in these activities or programs.

I further agree to indemnify, hold harmless and defend FAMILY CHRISTIAN CENTER and/or REFUGE PRODUCTIONS and the other released parties from any and all claims for injury(ies), damage(s) or loss(es) which I may have or which may occur to me on account of my participation in these activities or programs.

I further understand and agree that the terms such as “participation,” “programs,” and “activities” referred to in this agreement include all exercises and physical movements of any nature while I am participating in the programs and further include the provision of or failure to provide proper instructions or supervision, the use of any and all machinery, equipment, and anything related to my use of the services, facilities, or premises involved in these activities or programs, and transportation to any or from any event.

I understand the nature of the activities and programs for which I am registering, and have read and fully understand this WAIVER, RELEASE AND HOLD HARMLESS AGREEMENT.  I further understand that any advisement or warnings of particular risks of the activities or programs that I subsequently receive will be incorporated by reference into and become a part of this agreement.

I hereby release FAMILY CHRISTIAN CENTER and/or REFUGE PRODUCTIONS of any responsibility or injury(ies), damage(s) or loss(es) to anyone for whom I am legal guardian or parent that may not be specifically named.

Code of Conduct Statement

As a participant in the Family Christian Center’s and Refuge Production’s Performing/Creative Arts Ministry, I understand and agree to the following:

• I will uphold a high standard of integrity and character. I will not post negative images or negative and inappropriate comments on social media.

• I will not use any electronic devices during rehearsals and/or performances.

• I will respect myself, other people, other people’s property, and the church property.

• I will be an encourager, not one who ridicules, makes fun of, or criticizes.

• I will deal peacefully with anger and disagreements.

• I will participate and contribute in a positive way in all rehearsals and/or performances and obey the instructions of the Performing Arts Team, staff member and adult volunteer.

• During rehearsals and/or performances I will show respect by not causing disruptions and not talking unless called on to respond.

• I will use language that reflects a Christian attitude. I will not use curse words or other foul language.

• I will maintain my witness by keeping my hands to myself.

• I will wear modest clothing. No clothing that is sexually suggestive or spiritually degrading. T-shirts or other clothing with evil, vulgar, illegal, or inappropriate content will not be permitted unless approved of for a specific scene by the Costume Dept. In addition there should be: no midriffs, no low-cut tops, no see-through clothing, and no sagging pants (no boxers or underwear showing) unless approved of for a specific scene by the Costume Dept.

• I will not possess a gun, long blade knife, or any other weapon that can cause bodily harm.

• I will not use, possess or be under the influence of alcohol, tobacco, marijuana or other illegal drugs at any rehearsals and/or performances.

• I will not engage in sexual intimacy or sexual harassment, and will not possess pornography at any rehearsals and/or performances.

VIOLATING ANY OF THE ABOVE MANDATES WILL RESULT IN REMOVAL FROM THE BUILDING AND/OR THE PRODUCTIONS AND/OR POSSIBLE ARREST AND/OR PROSECUTION.

I have read the Family Christian Center’s and Refuge Productions Performing/Creative Arts Ministry Code of Conduct and agree to abide with each statement.
